When the ball goes dead
red card for the keeper - for deliberate impeeding a definite goalscoring chance (presuming that the ball was close enough to be considered to be playable by the players - if the ball is yards away it was no longer a goalscoring chance - then depends on the severity of the foul - probably still a red for serious foul play)
possible yellow for the forward for leaving the field (not to get too technical, but it kind of depends on how far over the line he was - wingers are never carded for stepping over the line if they are on a run, also the yellow card is normally reserved for ENTERING the field of play without permission, so may not be applicable here).
Restart from wherever the ball went out of play (or if the ref stopped play with the ball in play, restart with a drop ball)
there can be no penalty as the infringment was outside the penalty area
other than that i'm not sure - i gave up reffing 9 years ago - so i wouldnt be sure of any changes in the rules since then
