Have a look at http://guitargeek.com/rigview/304/ for a vague reconstruction of Hooky's circa-JD setup. The unknown effect pedal is probably Clone Theory by Electroharmonix, I've certainly heard Hooky mention this pedal before, but unsure when he started using it. You can get an approximation of it with any Bass Chorus and/or Flanger wound right out, also add a little distortion for drive.
As to what he has in the mysterious but massive rig seen in 316, not sure. Definately lots of rack f/x...

I never bothered with trying to spend a load just to get a Peter Hook bass sound. You can do it for less than £100 with a 'Zoom' bass effects pedal - programmable and actually includes a 'Hooky' sound as a pre-set effect.
However, you can add stuff to that if you're not satisfied...
